Insurance and Payment Information

  • Insurance Networks

    We are currently in-network with:

    Medicare Part B
    Blue Cross Blue Shield PPO
    Worker’s Compensation

For all other insurance plans, payment is due at the time of service by cash, check, credit card, or HSA/FSA cards.

We are happy to help you check your out-of-network benefits.

Please note: Telehealth and Ergonomic services are not covered by insurance and are offered on a private-pay basis only. Payment is due at the time of service.

  • Out-of-Network

If we are out-of-network with your insurance, we can provide a superbill—an itemized receipt that includes the date, type, and cost of services—for you to submit to your insurer. While reimbursement is not guaranteed, many plans offer partial reimbursement for out-of-network services.

    What is a Superbill?

A superbill is a detailed receipt that includes all necessary information about your treatment. If we are out-of-network with your insurance provider, this may help you recover a portion of your out-of-pocket costs by submitting it for reimbursement.

  • Payments

We offer flexible private pay options for patients who are uninsured, have high-deductible plans, or simply prefer not to use insurance. Telehealth appointments are also available as part of our private pay services for added convenience and accessibility.

    Payment Methods

    We accept:
    Cash
    Checks
    All major credit cards
    HSA (Health Savings Account) cards
    FSA (Flexible Spending Account) cards
